Forms & Policies

Discrimination & Bias

We are committed to creating a safe and inclusive learning environment for all. If you experience, witness, or are made aware of an alleged incident of bias, harassment, retaliation, hate, or discrimination, please report it immediately.

For the purposes of this form, acts of bias, harassment, retaliation, hate, or discrimination will be referred to collectively under the umbrella term of “discrimination.”

Please see the following links for the reporting form and more information:

  • Staff Only - REQUIRED:
    (1) Notify a school administrator.
    (2) Staff or building administrator completes and submits this form.

    All Others (not WPS employees) can either:
    (1) Notify a school administrator and/or
    (2) Complete and submit this form.

    If a district or building administrator is the alleged aggressor(s), reach out directly to Caroline Han, Director of Diversity, Equity and Belonging or Betsy Gavron, Assistant Superintendent.

    • The Director of Diversity, Equity, and Belonging, Assistant Superintendent, and Principal will be notified automatically via email;
    • Other administrators, such as the METCO Director, Director of Student Services, or the Title IX Sex Discrimination Coordinator will be contacted, if applicable;
    • If the incident has not been resolved and requires further investigation, an Investigator (typically, a building administrator) will be assigned.
  • You can submit this report anonymously. However, it may impede the investigation. Also, in most cases, disciplinary action cannot be taken against an alleged aggressor solely on the basis of an anonymous report.

Bullying and Reporting

  • Students, parents or guardians, and others may request assistance from a staff member to complete a written report.

    Oral Reporting: Oral reports made by or to a staff member shall be recorded in writing onto an Incident Reporting Form.

    Anonymous Reporting Links: Reports may be made anonymously using the anonymous reporting form available on each school’s web site.

    Written Reporting: Use of an Incident Reporting Form is not required as a condition of making a report.  Written reports can be delivered to any staff member.

  • A staff member will report immediately to the principal or designee when he/she witnesses or becomes aware of conduct that may be bullying or retaliation.  The requirement to report to the principal or designee does not limit the authority of the staff member to respond to behavioral or disciplinary incidents consistent with school or district policies and procedures for behavior management and discipline.

  • All reports of bullying will given to a building administrator who has an obligation to investigate and take the following actions in a timely manner:

    • interview all students involved
    • collect evidence including digital or hard copies of electronic communication web pages, notes, etc.
    • review previous disciplinary records of all students involved
    • notify parent(s)/guardian(s) of all students involved

Reports of bullying or retaliation may be made by staff, students, parents or guardians, or others, and may be oral or written. The District expects students, parents or guardians, and others who witness or become aware of an instance of bullying or retaliation involving a student to report it to the principal or designee.

Reports may be made anonymously, but no disciplinary action will be taken against an alleged aggressor solely on the basis of an anonymous report.
